About this role

James A. Haley Veterans' Hospital is looking for a Senior Social Worker Program Coordinator who will be assigned to the HUD-VASH program. This position works with an interdisciplinary team and provides screening and consultative services to the team and other staff regarding the psychosocial and homeless needs of veterans and the impact that homelessness has on their well- being. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning.

Duties The selected Social Worker program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for the facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. Additional duties include, but are not limited to; Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Incumbent meets the needs of customers while supporting VA MIssIons. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Incumbent provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the consumer constructively and appropriately. Adhere to Service Line Agreements, (SLA) Standard Operating Procedures (SOP) for documentation. Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, GARF, and/or revenue sources providing funding to VHA. Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in the discharge planning process both within the facility and for potential placement and/or aftercare programs/facilities. Adhere to SLA, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post discharge follow up. Provide individualize care coordination and discharge planning highlighting patients strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts. Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress. The GS-12 Social Worker Coordinator performs duties independently. The incumbent reports to the assigned Supervisor in social work service and to the Assistant Chief / Chief of Social Work. Incumbents may receive day to day oversight from an Assigned POC on their matrix/assigned teams, and this individual may be from another discipline. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensed: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Senior Social Worker, GS-12 Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Assignments: For all assignments above the full performance level, the higher- level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration, which includes consultation, negotiation, and monitoring Knowledge and ability to write policies, procedures, and/or practice guidelines for the program Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s) Ability to provide training, orientation, and guidance within clinical practice Keeps track of data and coordinates caseload size of staff in the HUD-VASH program. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area Social Work Coordinators are capable of meeting all KSAs for GS 11 social work qualification standards Preferred Experience: HUD-VASH experiences Data tracking experiences Excel Workbooks Reference: For more information on this qualification standard, please visit https://www.va.gov/ohrm/QualificationStandards/.
