SCHEDULE: Tues, Thurs, Sat, 9 - 5pm; Wed & Fri, 12pm - 8pm
As a Residential Coordinator, you will make a meaningful difference in the lives of individuals living in a group home setting. You will support clients in achieving their Individual Service Plan (ISP) goals, developing essential daily living skills, managing psychiatric symptoms and physical health needs, and becoming more independent. You will be an important partner in empowering clients to lead active, meaningful, and increasingly independent lives.
As a Residential Rehab Coordinator, your responsibilities will include:
* Advocating for your client caseload * Developing, implementing, and monitoring treatment plans * Overseeing medication management * Managing fiscal responsibilities related to funds * Serving as the primary liaison for service providers, families, and guardians * Utilizing an electronic health record system for client documentation * Entering notes into Avatar and reviewing notes from other residential staff for clarity and accuracy * Navigating computer programs for tasks such as medication sheets and doctor visit forms * Reporting incidents to the Site Manager and other providers while ensuring confidentiality
Requirements:
1. High school diploma and a minimum of one year of relevant experience; additional education in a healthcare-related field towards an Associate's or Bachelor's degree is preferred 2. Excellent communication and writing skills 3. Effective time management and the ability to organize multiple priorities 4. Ability to remain composed and de-escalate stressful situations 5. Good judgment and problem-solving skills 6. Ability to foster a teamwork environment and serve as a role model for colleagues 7. Capacity to build therapeutic relationships with challenging clients 8. Computer proficiency and the ability to learn to use an electronic health record system 9. Must possess a valid driver's license, a safe driving record, and access to a personal vehicle

Compensation is based on candidate experience, qualifications, and certifications and licenses. Annual ranges listed are based on a full-time, 40 hours per week position.
